Use the following instructions to file your Vacation Claim.

Step 1
Download a Vacation Application online under the appropriate areas: Deep Sea (download) or Great Lakes (download).

 

Step 2
Complete the Vacation Application in its entirety and verify that you have the appropriate documentation required to submit your application.

Step 3
Submit your application (in person, email, fax, regular mail or portal).
In Person: 2 West Dixie Highway, Dania Beach, FL 33004 - An appointment is required to submit your claim in person.
